Puppy Policy 

Congratulations on choosing a new puppy for your home. Before moving forward these are some steps and policies that we have in place to help you have a smooth transition from our home to yours. 

What to expect 

• The first step in purchasing a puppy from Bollenberg Farm LLC is to fill out the puppy application form. 

-Please be sure to state which puppy you are interested in on the application. If you are interested in a puppy that has been reserved, we encourage you to still fill out the application as it is possible he or she may become available. 

-We will reach out to you within 48 hours after the puppy application is filled out. If there is an error where you cannot submit an application, please contact us and we will help get it sorted out. 

• The second step will be to fill out the puppy contract where the deposit of $200.00 will be due to hold your puppy. 

• Puppies will be going home with a puppy package which includes:

-CKC Registration Papers

-Ages Stages Packet 

-Puppy Problem Solver Cheat Sheet 

-Small Fleece Blanket with Mothers' Scent 

-Vaccination Records and Vet Check 

-Teething Chews

  -A Toy
